Pronation refers to the inward rotation of the foot during gait. A small degree of pronation is necessary for appropriate shock absorption and the dynamic motion of the foot, however, an excessive degree of pronation at this level will go on to result in internal rotation of the leg and hip. This results in the knee being thrown into a valgus position and the pelvis into a forward tilt. As the legs, hips and pelvis rotate, the joints will become misaligned, leading to an overload of key muscle groups, which become overactive in order to restore balance and function during stance and gait. Over time, this overuse can lead to pain anywhere from the foot up to the level of the lower back.

Another common cause of lower back pain is having a leg length difference. Having uneven leg lengths results in the longer leg over pronating at the foot, to compensate for the poor position in which the hip is placed due to the discrepancy. While this method of compensation is helpful for returning the hip into a more balanced position, the over pronation starts to create muscular imbalances and throws other joints out of their neutral alignment.
